What Happened to Myles Garrett? Latest Update on Rams Star’s Injury and Why He’s Not Playing in Week 5 vs. Bills

Myles Garrett will miss the Los Angeles Rams’ Week 5 matchup against the Buffalo Bills as he continues to recover from knee surgery. Garrett underwent an arthroscopic procedure in mid-September and was placed on injured reserve, forcing him to miss at least four regular-season games.

Although the Rams’ defensive linchpin is making progress, head coach Sean McVay has indicated that the team will take a cautious approach before bringing him back.

What Happened to Myles Garrett?

Garrett dealt with persistent knee swelling and irritation during training camp but played through the issue in the Rams’ season-opening 27-7 loss to the San Francisco 49ers. However, he later admitted that he was not fully healthy, describing his performance as the “worst version of myself.”

The knee problem eventually required arthroscopic surgery in mid-September, following which Garrett was placed on injured reserve, making him ineligible to return until at least the next four regular-season games.

The Week 5 matchup against Buffalo marks his fourth consecutive missed game. While he is making progress in his recovery, his return date remains uncertain.

Sean McVay Provides Update on Garrett’s Recovery

McVay has offered an encouraging update on Garrett’s recovery, but made it clear that the Rams will not rush their defensive star.

Speaking on Saturday, McVay said, “His surgery went really well. He’s making good progress. As far as what that timeline looks like, it’s kind of week-to-week in terms of how he feels. But I would say it’s more likely that we do not start his clock next week.”

McVay’s comments suggest Garrett could miss more time beyond Week 5. The Rams want to make sure he is fully ready to play at his best before bringing him back, rather than rushing his return.

Rams’ Defense Steps Up Without Garrett

Garrett joined the Rams in a blockbuster offseason trade from the Cleveland Browns after setting the NFL’s single-season sack record with 23 sacks in 2025. His absence has left Los Angeles without one of its biggest defensive playmakers.

However, the team’s secondary and adjusted pass rush have helped fill the gap. Cornerback Trent McDuffie, acquired during the offseason, has been a key part of that effort.

McDuffie has allowed a completion rate of just 26.7% in coverage, recorded seven passes defended to lead all NFL cornerbacks, and given up only four receptions on more than 10 targets.

His performances have helped the Rams remain competitive while Garrett works his way back to full health. The Rams defense ranks fourth in the league with a 79.8 PFN Defense Impact Score.

Los Angeles enters Week 5 with a 2-2 record after earning a 24-20 win over the Philadelphia Eagles. The Rams will look to build on that victory against Buffalo, even as they continue to wait for their star defensive end to return.
